Congratulations on your recent graduation!

In order to receive your professional LSVT^® certificate and be registered as an LSVT Certified Clinician in our LSVT Global Clinician Directory, you must:

1. Complete the information form below.
2. Provide proof of graduation; a scanned copy of your diploma or transcript is acceptable. Once you submit the form below you will receive an automated email. You may respond to that email or send an email directly to info@lsvtglobal.com with your proof of graduation and/or applicable license attached.
3. Log in to your Clinician Account to update your employment and other pertinent information, including whether you would like to be listed in the LSVT Certified Clinician directory.
